A MIH/CHP program can help patients who are struggling to effectively access the healthcare system. They may be utilizing EMS and 911 for all of their healthcare concerns. CHP can help patients learn to utilize the community resources available while learning how to understand and manage their chronic medical conditions. This partnership empowers the patients to take control of their healthcare journey.
